%X Chronic heart failure remains one of the leading causes of morbidity and mortality worldwide, characterized by complex neurohormonal, hemodynamic, structural, and metabolic disturbances. This article examines modern clinical pharmacological strategies used in CHF treatment, with emphasis on neurohormonal modulation, RAAS blockade, beta-adrenergic suppression, mineralocorticoid receptor inhibition, metabolic therapy via SGLT2 inhibitors, and management of congestion using diuretics. Advanced therapies—including ARNIs, sGC stimulators, myosin activators, and anti-fibrotic agents—are analyzed in the context of evidence-based clinical guidelines. Phenotype-specific and personalized approaches for HFrEF, HFmrEF, and HFpEF are also addressed.
